Updates and Reminders
The FCSP Caregiver Satisfaction Survey's goal is to capture caregivers' opinions related to the assistance provided and the outcome of services as a result of the FCSP assessment. The information gathered supplements EOEA's and local agencies efforts to evaluate programs and assess on-going improvements and quality assurance.
Who should receive the Caregiver Satisfaction Survey?
- All caregivers that have received counseling, supplemental services or respite services, and
- All caregivers having been assessed using the FCSP Assessment Tool.
- Caregivers should be informed at the close of the assessment/counseling that they will be surveyed regarding their opinion of the FCSP services
When should the Caregiver receive the Survey?
- A minimum of 6 to 10 weeks after the initial assessment is completed.
How should the Survey be conducted?
- The survey can be conducted by phone, mail, email, or in-person. Please enter the information using this link: https://app.keysurvey.com/f/401147/44d6/
- Local programs can also give caregivers the link to the survey if caregivers are comfortable completing the survey online. The link for consumers only is: https://app.keysurvey.com/f/694475/7b29/
- Please enter all survey data for FY 2016 by November 15, 2016.

Who should conduct the survey?
The survey MUST be conducted by any agency staff OTHER than the Family Caregiver Support Specialist that worked with the caregiver.
Every quarter, the person in charge of tasks related to the FCSP Satisfaction Survey should enter the number of surveys administered during the quarter in the Administrative Survey. This survey helps us assess the rate of return of Caregiver Satisfaction Surveys. The new Administrative Survey allows those local programs involved in the pilot enter information.

Quarter 1 (Satisfaction Surveys administered from 10/1/2016 to 12/31/2016) - Please complete Administrative Survey by 2/1/2017
Quarter 2 (Satisfaction Surveys administered from 1/1/2017 to 3/30/2017) - Please complete Administrative Survey by 4/15/2017
Quarter 3 (Satisfaction Surveys administered from 4/1/2017 to 6/30/2017) - Please complete Administrative Survey by 7/15/2017
Quarter 4 (Satisfaction Surveys administered from 7/1/2017 to 9/30/2017) - Please complete Administrative Survey by 10/15/2017
